The objective is to create awareness of the topic “Social Determinants/Population Health” by defining the topic, understanding the community needs, become aware how employers, health departments, non-profit organizations and community groups are involved and understand the population health, healthcare model.

This program has been developed and is presented locally by the Midwest Chapter of the American College of Healthcare Executives. As an independent chartered Chapter of the American College of Healthcare Executives, Midwest Chapter of the American College of Healthcare Executives is authorized to award 1.5 hours of Face-to-Face Education credit toward advancement or re-certification in the American College of Healthcare Executives.
