Registering for this program is a two-step process.

2) You must register on the ACHE platform to have access to the login credentials and credits. There is no additional cost associated with this second step. If you are NOT an ACHE member you will “create an ACHE.org account”, but you don’t have to become a member.


USE OF MOBILE DEVICES: If attendees are using an iPad or other mobile device, they may be prompted for a passcode to enter your chapter event. The passcode is ACHE (case sensitive). This passcode is included in the confirmation emails registrants receive upon completing the registration process on ache.org, so please make sure to share the passcode in any communications.


The panel will discuss how their innovative primary care model improves accessibility and encourages a collaborative and coordinated approach with patients and providers.

Jay Thaker, MBA, MPH, MHS, Director of Population Health, Oak Street Health

Oak Street Health is a network of primary care centers for adults on Medicare. Jay’s work focuses on building and scaling new capabilities that reduce the total cost of care for their globally capitated population while maintaining or improving quality, patient experience, and provider satisfaction. His team’s work has focused on a new care model for late-stage chronic kidney disease patients, expanding access to cost-effective cardiac diagnostics, and improving pharmacy services in the primary care setting. Prior to Oak Street Health, Jay worked at Cleveland Clinic, helping to grow the organization’s international footprint through greenfield development, partnerships, and acquisitions. Jay is a 2014 MPH graduate of the University of Michigan Health Management and Policy program.

Samantha Sharon, Chief Operations Officer, Ascend Medical

Samantha joined Ascend Medical in June of 2020 and has 30 years in the healthcare field. Samantha most recently served as the Chief Compliance & HIPAA Privacy Officer for Benevis.

Ascend Medical has allowed Samantha to work with a management team of innovative thinkers to create a company with a focus on “patient first healthcare”. We know that patients are consumers and have a choice, it is our goal to be that choice to reduce ER and Urgent Care needs and ensure all care is consistently based on their health history and needs.

Ascend Medical launched in November of 2020 and is a membership-based healthcare service offering primary care, pediatrics to the patient through mobile diagnostics and all services including behavior health available 24/7 through virtual visits to address all healthcare concerns.

Brian Madison, FACHE, FHFMA, President/Founder, The HCS Group

The HCS Group initially began as a management service organization (MSO) specializing in mobile MRI and PET/CT services. Shortly thereafter, Brian expanded the services to include “turn-key” outpatient imaging clinic management services.

Most recently, Brian founded Ultra X Imaging, which specializes in coordinating state-of-the-art x-ray and ultrasound services with hospitals and providers throughout the post-acute care continuum with a focus on reducing unnecessary emergency department visits and readmissions.

Brian is a Fellow of the American College of Healthcare Executives, a Fellow of the Healthcare Financial Management Association, and is a member of the Industry Relations Board American Academy of Home Care Medicine. In his free time, Brian enjoys competing in Ironman triathlons. 

You can earn 1.5 face-to-face credits for attending the entire Zoom program. This program has been developed and is presented locally by the Midwest Chapter of the American College of Healthcare Executives. As an independent chartered Chapter of the American College of Healthcare Executives, the Midwest Chapter of the American College of Healthcare Executives is authorized to award 1.5 hours of Face-to-Face Education credit toward advancement or re-certification in the American College of Healthcare Executives.
